This is a myth, it’s not at all difficult to fake if you are using condoms for example (who checks inside them?). Plus most men produce some ejaculate as they go along, and so do women of course, so there is usually something fluid there. Also ejaculation and orgasm in men are not quite as connected as people think, you can have one without the other.
