I used to work with my dad on various projects and he would always be pissed at me due to my poor performance. I thought it was just him, but once I started working I noticed that they began to happen there as well. It's these four things:

1. Usually don't understand directions given to me to the point where I either end up doing the task that I was assigned to incorrectly, have to ask my supervisor or boss what to do again or have to be verbally told what to do when I'm seen messing up. I'm often afraid to ask what I'm supposed to do if I’m unsure of the directions because I don’t want to irritate my boss or supervisor anymore (this leads to more mistakes.)

2. Usually forget what the directions are for a task assigned to me (either because I couldn’t hear them or I wasn’t paying attention) and I either end up doing the task that I was assigned to incorrectly, have to ask my supervisor or boss what to do again or have to be verbally told what to do when I'm seen messing up. I'm often afraid to ask what I’m to do if I’m unsure of the directions because I don’t want to irritate my boss or supervisor anymore (this leads to more mistakes.)

3. I stand around or wander around the job site not knowing what to do next because my boss or supervisor haven't told me what to do next (or the first two issues come into play.)

a. When I am to be given something to do, I have to be given instructions immediately after I finish one task (and repeatedly after that until the work is completely done for the day) or one simple and straightforward task that takes a long time to do so I don't have to bother my boss or supervisor and constantly ask what to do next often.

4. I easily get distracted and/or my mind wanders and my work slows, stops altogether or I make tons of avoidable mistakes that wouldn’t have happened if I was paying attention and my supervisor or boss often get irritated that they have to keep telling me to speed it up or keep working.

Hello,
I have had the same things happen to me on the various jobs I've held. My supervisors would be plain disgusted with me. I would often hear this: "I've told you this before".
I've had these problems since age 8 or 9 and it never got better. Next week I go to my doctor to get the proper diagnosis. All the signs are there. I also have such a difficult time keeping anything organized especially my apt.
You might try to seek help from a doctor then bring that list you typed & show it to him/her see what the prognosis is.
