Here's what I'll throw out there. It's true for me. Have no idea if it is for anyone else.

Obsessive rumination is typical with BD. So you're going around and around in your mind about medication. Around, around, like a hamster on a wheel. As soon as you think you might be close to a resolution - there you are, right back to ruminating, questioning, endless questioning. Essentially your mind is stuck in gear.

So how do you know if a med is working? One way to know is that you feel stable. You feel like the ground you're standing on is level. Nothing is suddenly going to fall out from beneath you, and you're not going to tip over.

Your mind isn't stuck in ruminating over the same set of questions, over, over, over, no break.

You know a med is working because you know it's working. You don't feel compelled to keep obsessively asking yourself, because you feel more confident about how you feel and what you're doing. "Yeah, this stuff is really helping me." And you can look back on the "before", compare, and see the difference between then and now clearly.

Even the best meds don't mean you won't have times of relapse. But those times are generally shorter and less intense. And sometimes you need to adjust meds to meet your current needs.

I don't know if this makes any sense, at all. I hope it helps answer your question a bit.
