There's actually a site called Mood Tracker at Mood Tracker - Help for depression, anxiety, and bipolar disorder. They have both a free option and premium option (for a bit of money). I think the free option has plenty of features. I used to use that for quite a while, but I've stopped tracking my moods daily. You can definitely print not only mood charts, but your med list (if you choose to enter it) and med history, and journal entries. I assume you can also print various things with the premium option. There is a forum there, but it's not active. The mood tracking allows people to kind of enter mixed state information. You can also include things like anxiety level, irritability level, hours of sleep, if you took your medication or not, or if you're menstruating. I think the premium has even more detailed tracking options, as well as more detailed tracking reports.

I'm not that familiar with mood tracking apps. I've trialed a couple in the past, but that was a long time ago. I didn't like them as well as the site I mentioned above.

Here is an example of a report I printed for information I entered way back in 2011. You can print by month, quarter, or year. 2011 is as far back as I can still print. You are also able to print a list of the medications (and doses) you are/were on at that time (if you entered them) and any journal entry from that time. If you entered a mixed state, two bars show for the same day (one as the high for the manic side, and the other for the low of the depressive side).
