She seems nice. She's going to taper me off Ativan and oral risperdal (will continue with the shot). I want to ask her for some PRN for psychosis, but I've had mostly bad experiences with pdocs and honestly I'm kind of afraid.
How do I muster up the courage to ask her? And which meds are good as a PRN for breakthrough psychosis if/when it gets too much?

I don't ask for PRN's just ask what your suppose to do when you have breakthrough symptoms.

Dr. W. tells me to add on a small dose of Zyprexa if I feel the need to do so. And I know that some people taken Seroquel prn.

Miguel'smom has a good idea, though...ask your NP what to do if you have breakthrough symptoms.

Zyprexa is good. Seroquel. Why is she tapering you off ativan?

Hopefully she is recommending a very slow taper. Rebound anxiety can occur and you might have to push through that for a while until you're off especially if you were taking it every day. The slower the taper the better.

How long were you on it?

She didn't give me a "go-to" in place of Ativan. I've only been on it over a couple months initially at 3mg/day and now I'm just at 1mg/a day at night.

I'm sorry...I didn't mean Ativan. I meant to ask if she gave you any ideas for a prn if psychosis breaks through?

Does a psychiatrist supervise the nurse practitioner? Can you ask to regularly see the psychiatrist as well as the nurse practitioner?
Do they have a complete and detailed med and medical history on you?

I'm not sure why you're afraid to ask your psych nurse, but if you are afraid of coming across as a druggie, don't be! Most PRN's for breakthrough psychosis are APs and such, with benzos occasionally used to treat secondary symptoms (such as anxiety and restlessness stemming from paranoia). But if you're afraid that she is going to do something you might not like, why not have a conversation with her? For example, "I occasionally have breakthrough psychosis, but I really like my meds as they are and I don't want to be prescribed something that will cause X, Y, and Z side effects."
