Hi, About 5 years ago I went on the poison called "Seroquel". The stuff has never done anything for me - except cause me to gain over 100lbs. I am down from a high dose to 25mg. So close to going off of it. I must lose weight. I am seriously concerned about my health if I stay at 250lbs. Of course, I know about diet and exercise - although exercise is pretty tough when I am in chronic pain (to the point of having difficulty walking) from carrying excess weight.

So. First off: Is there any prescription medication that assists in weight loss?

Second: Years ago, I was just a few lbs over my ideal weight. I was on an exercise program, eating very carefully, and was using ephedra (ma-huang), at that time touted as a great way to boost performance and lose weight. I felt fine, no obvious problems with ephedra. I reached my weight goal with no problem. Shortly after I stopped taking the ephedra it was pulled from health food stores because supposedly it could cause cardiac damage. It is now illegal in the U.S.

The weird part? Ephedra has exactly the same potential side effects and possible dangers as Seroquel does.

Since then, I am completely out of touch with herbal remedies for weight loss. Any suggestions?

Oh, the hazards of weight gain from psychotropic meds. I am sorry you have gained so much weight from the Seroquel. Glad you are weaning off it.

There are currently some FDA approved drugs for weight loss but most of then have side effects so you would have to look at that carefully before diving in. I wonder if you might revisit what you diet was when you lost weight and maybe copy that. That was part of the equation so at least implementing that part would probably help. Too bad you are unable to exercise because boosting your metabolism that way instead of using a stimulant would be safer. If you can get some of the weight off through diet, it might become easier to begin a walking or exercise program.

Getting started is always the hardest part. Maybe a coach or an app for your phone would be helpful. I hope you find something that works for you.

Thank you, LL. I can exercise some, just not as much as I'd like to - for example, running would be too painful, and I just don't feel safe cycling as I used to when younger.

My diet was very important when I lost weight earlier in my life - the problem is, I had less than 10 lbs. to lose, and I was using ephedra.

I asked my pdoc about the availability of prescription weight loss meds yesterday; she was vague and gave me an answer that didn't seem (as usual) to take my desperate need to lose weight seriously.

I dunno. I'm kind of thinking that, along with going off of Seroquel, I should just go ahead and try some kind of athletic enhancer that includes ephedra in it. When I'm this desperate, but not being taken seriously by MD's I feel like I have no choice but to step away from the traditional medical realm.

When you say running would be too painful, do you mean any running at all? You could try starting a run/walk program, like Couch to 5K, where you alternate short bursts of jogging with running. It's a 9 week program, and it starts with something like minute and a half walking alternating with 30 seconds of jogging. You can repeat weeks/workouts until you feel comfortable moving forward.

My cousin was on Seroquel, and she gained weight. She warned me when I went on it, so I started counting calories immediately and put myself on a pretty restrictive diet. Fortunately, I was able to keep my weight the same. I was experiencing cravings like crazy, though, and saw how easy it would have been for me to gain weight if I hadn't been watching myself like a hawk. You could try a calorie counting app, and watch what sort of nutrients you're getting.

I know you're feeling desperate, and these solutions are a little slow moving, but they might be worth a shot. I hope you find something the works, and please be safe.

Oh, no way can I run at this point. I have painful Achilles tendonitis...if it isn't wrapped, walking is very painful. I'm working on just being able to walk for longer stretches - by that, I mean, walking around the grocery store or a department store is all I can do. By the time I am walking to my car, I'm limping. By the time I get home I'm in serious pain.



I used to love to run (jog). But that's the vicious cycle...I weigh too much to be able to job, so because I cannot jog (or whatever aerobic activity), how can I lose weight?

If I could alternate between walking and running as a strat to get back in shape, I'd be out there right now.

Until I was 41 I was still dancing ballet. So you can imagine how thin I was, and in excellent shape. I still use the stretches I've used for 40 years, which is great for retaining flexibility and helping my peace of mind, but stretching hardly supports weight loss.



A big problem since mid-June has been the wildfires here in Calif. The smoke is BAD. I spent as little time as possible outside during the summer because I quite literally couldn't breathe well. I had just started taking some short walks - and now here we are again, smoky air and ash falling. The little bit I was outside today I was coughing, my eyes burning and tearing. These past 5 months have been a hell here.

You're fortunate. I had absolutely no idea, when I started taking Seroquel, that I would gain weight like this. I have always eaten a healthy diet. I don't overeat. A few years ago I was still able to bicycle. I sold my car and cycled everywhere for 6 months. To work, stores, cafes, post office, anywhere I would have driven. Had I not been on Seroquel my weight would have dropped to the point at which I was too thin. During those 6 months I didn't lose 1 lb. In fact, I gained a bit of weight. The "experiment" was extremely discouraging - to say the least.

I know you're feeling desperate, and these solutions are a little slow moving, but they might be worth a shot. I hope you find something the works, and please be safe.

Ah, yes, I understand now. I think the first order of business is to take care of the Achilles tendinitis. Lots of RICE-ing (Rest, Ice, Compression, and Elevation), but it sounds like you've got that covered already.

I have a similar cycle with running and my mental health. I run to battle my depression, but if I get depressed, I have no motivation to run. Unfortunately, I got injured training for my last race, so I haven't been able to run since Labor Day, and the depression is coming back.

I was also a ballet dancer, but I quit training pre-professionally in high school. While I was living in the Midwest, I was taking some adult ballet classes fairly regularly before switching to yoga (a little easier on the body), but we moved back east and I just haven't gotten back into either activity. I completely understand the body issues ballet creates, and that was the exact reason why I was counting calories on Seroquel (and why my cousin warned me. And yes, I agree, I was extremely fortunate. It would have wrecked me otherwise).

What about investing in a stationary bicycle and a heart rate monitor? It should be easier on your joints (although, I'm not sure about the tendinitis). With a heart rate monitor, you can, well... monitor your heart rate to really push yourself while your riding to make sure your getting a good work out on the bike. If you have a bicycle you like, you could always get an indoor bicycle trainer. That way you don't need to buy a whole new stationary bike, and once you feel comfortable riding outside again, it's easier to store a trainer than a stationary bike. I'm wondering if the reason you gained weight riding the bike around town was because you were building muscle?

I had a friend who was prescribed metformin for weight loss. It is normally used to treat diabetes, but apparently can also be used for weight loss. You could try asking your doctor about it.

If walking is too painful, is there anyway you could get access to a pool and try swimming? It is excellent exercise and much easier on the joints.

I totally forgot about swimming. I personally hate swimming (dog paddle only), so I never think of it. I'm glad you mentioned it!

Topamax maybe?

This isn’t geared towards anyone on PC but sometimes people have no choice when it comes to meds and weight gain. There’s lamictal and geodon is decent. There’s Wellbutrin if you don’t have anxiety issues. But sometimes there’s just no other option besides meds that cause weight gain. I was there personally when I was a teenager with many meds including Seroquel.

A girl I know was very underweight from an eating disorder. She was going inpatient multiple times, getting kicked out of multiple ED treatment centers and was having health issues. As a last resort her doctors put her on several meds that cause weight gain. Now she is morbidly obese but her mental health is so much better. Sometimes the ones that cause weight gain are the only options.

Again, not directed towards anyone here. Just a personal rant.

I'm in the same position but not from seriquel. My doctor tried to work with me but I'm in the but no that won't work. He suggested swimming but the only place here has no HC parking near the door...the place is not really assessable at all and just getting into the building with my back pain is a problem. Not to mention I'm very self counsels about my weight and using a locker room! He did try to Rx but my insurance wouldn't cover the meds which are the most effective. I did go on metaformin but only lost 10 lbs then regained 5. I've kept track of my eating and while not the healthiest my accual calories aren't that high but my metabolism sucks big time. Pain keeps me from wanting to do much like walking, after a half block it gets painful quick. I don't even go to malls any more cause there's not carts to hold on to and the malls are just too big. So in a viscous circle I do less which slows my metabolism even more.

I don’t really understand it either. You’d think she would be worse. But She’s just doing so much better then she was before. Her meds have really helped her. She says she doesn’t care about her weight anymore and is happy with how she is.

I lost 30 lbs one summer on wt watchers w no exercise. You have to like vegetables and I do. I skipped the meetings. Plant based diets are very good for yr health and supposedly they prevent Alzheimer’s, so I’m going to try. The problem is that veggies don’t keep that long in the frig and I don’t want to spend all my prep time chopping. So lazy am I!
Have you tried a medically supervised diet ? They often use liquid diets like smoothies, at least at first. They are very strict.

Yes, we have them here, too. They are stimulants, which scares me a bit with regard to anxiety. My pdoc mentioned that stimulants can cause a manic episode, but so what? I mean, I don't get so manic that I'll do something crazy...for one thing, I have $10 in my bank account, so nowhere to access money to fly to Korea or whatever. And I won't do anything that will leave my cats in danger.

Anxiety scares me, but if that happens, I can just stop the pills.

If my GP won't prescribe anything to help, I'm going to look into OTC help.

Laurie,

the options are as follows:

Naltrexone, Topamax, Zonegran, Wellbutrin. That, assuming that you were taking Seroquel for Bipolar and cannot take combined medications with Topamax because the other ingredient can cause mania.

So, of the above, the most effective is Zonegran. Unfortunately, I could not tolerate it - it produced severe mania and a skin reaction on my face.

I could not tolerate Naltrexone because it produced a depression.

I could not tolerate Wellbutrin because it caused insomnia.

I am, knock on wood, tolerating Topamax second time I am trying it, and it is an amazing medication in that it helps the remainder of my racing thoughts that even Zyprexa could not control and has returned my Zyprexa-propelled appetite to normal even at a small dose (and I am slowly titrating Topamax up). I have lost a tiny bit of weight without exercise, but it is too soon to speak of results since I only started recently.

Naltrexone and Wellbutrin, while causing side effects, did nothing to the enormous and pathological hunger that Zyprexa caused.

When I tried Zonegran, I was not on Zyprexa - I was on Geodon, so I took it hoping to prevent migraines and to lose weight rather than to control appetite and it caused mania so fast that I did not have time to notice its effect on appetite.

These four are the options among the neuropsychiatric medications. Zonegran and Topamax are originally anticonvulsants; both are used in migraine prevention. Naltrexone is primarily a drug for addictions. Wellbutrin is a antidepressant in a class of its own. Topamax and Wellbutrin are combined in a weight loss medication called Contrave, but I always prefer having separate medications because you can fine tune dosage.

I deeply regret going off Topamax when I tried it first time in 2012 - had I not done it, I would not have gained 85 trying various other meds. I stopped it when I did not have a consistent pdoc and was myself uneducated, so when I started having side effects, it did not occur to me to try lower dosages, read up etc. Now I know what i was doing wrong, so here goes:

With this drug, it is imperative to drink copious liquids. Meaning - drink a lot of liquid and then some more. Drink as much as you can and then some, to keep it safe. This will prevent dizziness and some other side effects. And, titrate up ever so slowly. Topamax comes in 15 mg sprinkle caps - you take 1 tbsp of apple sauce, open a capsule, sprinkle the contents onto apple sauce, eat it at once, and then drink some liquid. Do that for a couple of weeks. If you can tolerate that, Topamax also comes in 25 mg tabs, so get those and take 25 mg at night and no sprinkle caps. If you can tolerate that, stop 25 mg tabs and go back to sprinkle caps, but take two - one at night and one in the morning. This way you will be taking 30 mg. Then 25 mg tab at night plus 15 mg sprinkle in the morning = 40 and finally 50, if you can tolerate it. 50 mg is as effective for weight loss as higher dosages, so this would be your goal. Go slowly - you have no emergency. What is out online about titrating by 25 mg once a week is for epilepsy when there is a pressing need to go up fast - you do not have it and please allow the body to slowly grow used to the drug. Topamax is effective in preventing weight gain from Zyprexa, and as weight-gaining antipsychotics go, Zyprexa is even worse than Seroquel, so it would make perfect sense for you to try Topamax. Of course, if you can tolerate Zonegran (my reaction to it was highly unusual, though not unique), even better - the mean weight loss from Zonegran in one study was twice that from Topamax. Worth trying. Wellbutrin is less effective than Topamax. Given the relative effectiveness, I would start with Zonegran - in most people it causes no neuropsychiatric side effects.

I think it is only fair that a problem caused by a pharmaceutical medication should have a pharmaceutical solution - think of it as an antidote.

I am even heavier than you are - about 262 lbs at 5'5'' and some of it, though not the majority, comes from Seroquel as well. My weight pre-meds was around 140.

Metformin did not help me lose weight when I was not on Zyprexa and did not help prevent weight gain when I was not on Zyprexa. My internist does not want me on it now. But some doctors routinely prescribe Metformin to everyone who takes Clozapine or Zyprexa.

I hope you find something that at least halts further weight gain. You certainly deserve a break!

—-I have found water aerobics to be fun and amazing—the only exercise program I have been able to stick to. LAFitness has a huge program adaptable to ones schedule. I try to keep fruit in the house or I am insatiable for carbs.
