So I now know the truth. If you fill out a survey from your medical clinic don't say anything bad about a doctor. Even if they made a mistake that affects your life by just not listening to you. I'm totally deaf in my left ear and have moderate hearing loss in my right ear. Now no ENT doctor in my area will touch me they will look that's it.

I'm not an expert on ears but perhaps a primary care physician or audiologist would be able to help if you've run out of ENTs?

Those surveys are supposed to be kept private. The system that provides my healthcare uses an independent third party to collect responses, evaluations, etc. I don't even have to sign it.

It's a shame if what you shared in a solicitation of your feedback is being told to your doctors and that you are being identified to them as having said such-and-such. That's a real breech of your trust.
