I went to the ER and I was given olanzapine (Zyprexa) 2.5mg to take 3 times a day.
I am worried about long term weigth gain, metabolic issues and current sedation, this is why I plan to ask my new pdoc to change it.

I am a CYP2D6 poor metabolizer

I cannot tolerate aripiprazol, risperidone, ziprasidone and old AP.

What is your experience with latuda or Saphiris?

I tried both latuda and saphris. With Saphris I needed to wander around a lot and latuda I don't remember what went on there, except that I have issues with a LOT of meds. I did try Zyprexa for over a year and all it did was make me gain weight and kill my metabolism and raise my triglycerides and give me pre-diabetes. I think you've made a good decision to stay away from that. How many meds have you tried? I've tried a lot, but thankfully I can tolerate Haldol. Seroquel works for me, but its got the weight gain aspect to it, too, though not nearly as bad as Depakote and Zyprexa I don't think.

Sapharis cause me a massive Asthma attack. Plus is plain tastes awful but I’m gonna assume one could get use to it ..

Latuda worked well for me no real side effects

I have tried most atypical AP except Saphiris, Latuda, clozaril and xepelion.

About typical, I have tried haldol and another one I do not remember.

I take Latuda and find it to be helpful and relatively weight neutral. It works well for me as an antidepressant in addition to my mother medications. It's very expensive, though, so not always the best option.

Is it possible to just take less olanzapine?

I say that because it is a good antipsychotic for me, meaning it got to the bottom of the psychosis really quickly.

I am now taking 2.5mg olanzapine (in addition to other things), but I am planning on going off of the olanzapine soon with my doctor's help.

WOW. 3 times a day?? I mean, 7.5 mg is a dose of Zyprexa, but 2.5 mg spread over the day is like... are they trying to turn you into a zombie?! Jeez!

These are good choices for someone who is a CYP2D6 poor metabolizer.

Latuda for me the first time was very nice. It stabilized my moods, my moods were more "fluid," and I was more "me." All I needed was 40 mg and that was the dose I took for a year and a few months. I actually lost weight when I started it, believe it or not... I was never sedated. Unfortunately, suddenly I got akathisia from it after so long from taking it (tardive akathisia, it's a legitimate things that happens apparently), so I had to switch to ziprasidone. However, these recent few times I've tried Latuda have not been so great. I have had to have higher doses (60-120 mg) and my depression has been much, much worse, and with greater side effects (parkinsonism being the main side effect as well as blunted affect). But I will tell you this... In the last month that I have been taking it, I have lost 25 lb (~11 kg) from not eating due to depression! lol

Saphris I have actually never taken as my main antipsychotic. I have only ever taken it for sleep. But from my experience, taking 5 mg at bedtime puts me to sleep super fast. It also has a very calming effect during the day the next day (if I have a panic attack, it kind of washes over me like a waterfall, I can't really explain it any other way...). Sometimes I need 10 mg (the target dose) to get me to really calm down and go to sleep. It is intended to be taken twice daily (10 mg 2x/day). Personally, I don't see how anyone could do that without being zombified because of how sedating it is. It is relatively weight neutral, I have never gained weight on it. It is capable of causing weight gain though. It is also capable of causing heart arrhythmia (QT prolongation, but rare). Also, it tastes very, VERY bad. It is supposed to taste like cherry, but it is very nasty. It's a sublingual tablet that you have to not eat or drink 10 minutes before or after you take it or else it won't be absorbed. Also, in my experience, if I take it too many days in succession, the hypnotic effect wears off (which would be good for someone taking it daily) but I start to get dysphoric and weepy (not good for someone taking it daily).

Also, the antipsychotics that are metabolized by CYP2D6... you could probably tolerate but you would need to take 50% of the dose. Aripiprazole for example, if you started at 5 mg, you would need to take 2.5 mg (or 2 mg since they make that dose).
