I am dealing with possible narcolepsy and also my anxiety has been terrible. I am having issues at my job with my anxiety getting in the way which then gets in the way of my ability to perform well. I also will leave early sometimes because I am so tired. I want to cut my hours from 20-25 a week to 15 hours a week. What is the best way to talk to my manager about this? Should I explain my issues or just say that I’m dealing with a lot of personal stuff?

Can you get a doctor's note reducing your hours? Might go over better.

I did tell him today. I just told him I was dealing with a lot of personal and health issues and needed my hours cut. He didn’t say much just agreed to do it. he had me write it down. He was in a mood today and I couldn’t tell if it was because of me or some other coworker who has been causing a lot of issues.
