Are there any protections in place for employees with mental health issues to ensure we can take time off work for medical/counseling appointments without penalty?

In the US this would fall under the employment clause of the Americans with Disabilities Act and you would need to make a request for reasonable accommodation. The only reason they could turn it down is if it placed undue burden on the employer. But you do have to disclose disability/illness and make the request. If you do not make the request and do so, then they would not be held accountable to the ADA.

You would need to talk to your HR department and ask how to make a reasonable request for accommodation.

Treatments I've done: CBT, DBT, Transcranial Magnetic Stimulation (TMS), Talk therapy, psychotherapy, exercise, diet, sleeping more, sleeping less...

I’d ask your work place. It’s hard to tell what each employers provide? In most places you get sick days etc Talk to your boss or HR (read employee manual too)
